Select from: PLAN, IN PROGRESS, ISSUE, or DONE for each item in the schedule.
 

This is how my team operates. Any item indicated as ISSUE requires comments and pics. All ISSUE statuses across all projects can be found thru a report or dashboard, so we keep our eye on getting it resolved.
